PRIVACY OF PATIENT INFORMATION AT TRANSCEND PHYSIOTHERAPY AND WELLNESS
The Personal Health Information Protection (PHIPA) outlines the responsibilities that Transcend Physiotherapy and Wellness has for ensuring the privacy of your personal information. Namely, Transcend Physiotherapy and Wellness will:
- Only collect information that is necessary for your care
- Share information with third parties only with your consent
- Comply with PHIPA legislation regarding the retention and destruction of personal information
Transcend Physiotherapy and Wellness will collect, use and disclose your personal information for the following purposes:
- To assess your health care needs and to deliver safe and appropriate care
- To allow members of the multi-disciplinary health care team to communicate regarding your treatment to prevent duplication of services and to collaborate in your care
- To enable communication with you during or after the period of your treatment
- To communicate with a third party with respect to policy coverage and procedures
- To complete and submit claims to third party payers
- To comply with requests from solicitors in cases of personal representation
- To invoice for goods and services
- To process credit card payments
- To collect unpaid accounts
Under the PHIPA, you have the right to access your personal information and medical record at any time during or after the course of your treatment. There is no charge to view your information and records; however; should you require a copy of your information, Transcend Physiotherapy and Wellness reserves the right to charge a reasonable and customary fee for copying and administrative expenses.
